What is the Commonwealth Home Support Program?

The  Commonwealth Home Support Program (CHSP) is an entry level program for people aged 65 years and overproviding basic level care needs and support for people to stay living at home longer. 

ACH Group provides a range of services to maximise people’s independence by providing high quality basic home support services including domestic help, personal care, social experiences, allied health, respite and transport. 

Price list and eligibility

If you are eligible, you are expected to contribute to the cost if you can afford to. We will discuss fees with you before arranging services. Learn more about the CHSP price list and eligibility.

How to apply

To apply, you will be referred for assessment to the Regional Assessment Service (RAS) who may conduct a face to face assessment before recommending services under the Commonwealth Home Support Program.  

ACH Group can help assist with lodging a referral for an assessment from My Aged Care on your behalf. 

We will request that a representative from My Aged Care contacts you to start the process.

My Aged Care will contact you to understand your needs and recommend services for you. This will include a phone screening and a face to face assessment, if required. 

If you would like ACH Group to provide your service(s), simply tell your assessor and they will contact us to arrange. Alternatively, ask for a referral code and call us back on 1300 22 44 77. 

We will contact you to determine when you would like your service(s) delivered.

Last week, ACH Group was excited to receive the national HESTA Awards Team Excellence Award in recognition of the our innovative Cycle Challenge – a program designed to promote wellbeing, social connection and community engagement amongst aged care residents.

The program was delivered in collaboration with Tourism SA’s Tour Down Under, the initiative engaged 210 residents across multiple ACH Group aged care homes to participate in a structured cycling challenge. Designed to enhance physical wellbeing, encourage social interaction, and cultivate a sense of purpose, the program reflects ACH Group’s commitment to support healthy ageing and community connection.

Residents collectively cycled 546 kilometres, with the leading home recording 1,304 minutes of active participation. These outcomes were made possible through the coordinated efforts of ACH Group’s Health and Social teams, whose collaboration ensured the program was inclusive, impactful, and aligned with ACH Group purpose of supporting older South Australians to live well.

This national recognition reflects ACH Group’s commitment to creating real experiences to support older people to live well, stay active, and remain connected to their communities.
