Home Care Package (HCP)

We understand that you may have a lot of questions to ask. Here you’ll find some of the most commonly asked questions about home care packages.

Home Care Packages are a government subsidised, flexible plan of care to meet your needs, and are designed to support you to stay living independently in your own home. 

A Home Care Package can be accessed when you need more than basic support, and offers a coordinated approach with access to a greater range of services, aids and equipment.  

An experienced care manager works with you to design and deliver a range of support and services to ensure that you get the most out of your package.  

ACH Group provide the following Home Care Packages: 

Level 1: Supports people with basic care needs
Level 2: Supports people with low-level care needs
Level 3: Supports people with intermediate care needs
Level 4: Supports people with high-level care needs 

To qualify for a Home Care Package you must first have had an assessment completed by the Aged Care Assessment Team (ACAT) and based on this you will be assigned a home care package level. 

To request an ACAT you must contact My Aged Care  or call 1800 200 422.

If you do not qualify for the subsidy, or your circumstances are geared more towards paying for the support you need yourself, we can develop a tailored Home Care plan with you that includes arrangements to pay as you go. One of our friendly, experienced care managers can meet with you in your home or a location of your choosing, to design a plan that works for you. You are welcome to bring a family member or friend along as well. 

An ACH Group Home Care Package can feature any combination of the below services and if we do not offer a particular product or service, we will arrange an alternate supplier of your choice.

At ACH Group, once we have established your care needs and/or created your support plan, we work with you around affordability to ensure you get access to the services you need and at the best value. 

A Home Care Package is appropriate when you need more than low level support, and offer a coordinated approach with access to a greater range of services, aids and equipment.

An experienced care manager is also on hand to ensure that the Home Care Package is meeting your needs. 

ACH Group does not charge a basic daily fee for Home Care Package services, however an income-tested care fee may apply.

If you are not eligible for a government subsidy or don’t want an assessment and would prefer to pay for the services yourself, we offer a ‘Pay As You Go’ option. 

When it comes to a Home Care Package, ‘Approved’ and ‘Assigned’ mean two very different things. 

If you have been assessed as needing a higher level of support by the Aged Care Assessment Team (ACAT), you will receive a letter stating that you have been Approved for a Home Care Package (HCP). At this point, if you wish, your name will be added to the National Priority Queue. 

It is important to note that after being Approved for a HCP there may be a waiting period before you will be Assigned a HCP. Only once you receive a letter from My Aged Care stating that you have been Assigned a HCP and given a referral code, are you able to access HCP funded services. 

If you are waiting and need services, please call ACH Group on 1300 22 44 77 as we may be able to help you access services under the Commonwealth Home Support Program, and there is also the option to fund your services privately.
