Our trained ACH Group Support Workers will bring an iPad into your home and assist you to connect with family and friends, giving you the opportunity to see them face to face online.
The platforms used will be the closest thing to having your loved ones in the same room as you.

What services are available under Support at Home?
ACH Group services are tailored to meet your unique goals and preferences, falling into three main categories – clinical supports, independence and everyday living
Your dedicated Care Partner at ACH Group will collaborate with you to choose the right mix of services from your approved list and adapt your support as your situation changes.

Access professional health services such as nursing care, occupational therapy, and physiotherapy, along with essential care management to support your health and wellbeing. 🩺💪
✅ Independence
Get help with personal care including showering, dressing, medication support, transport, and short-term respite care - supporting both you and your carers to live well and independently. 🚿🚗
✅ Everyday Living
