Once your care plan is in place, your dedicated Care Manager will work alongside you to ensure your services continue to meet your needs over time.

Your Care Manager will be:
✔️ Your single point of contact for any requests or issues.
✔️ Providing information, advice and referrals as needed.
✔️ Conducting an annual review to ensure your services and supports are meeting your needs and preferences.
✔️ Providing ongoing management of your package funds, and adjustment of services and supports if your needs change.
