Services – Support at Home

The Support at Home program is designed to provide flexible and personalised care, helping you to stay independent and connected in your home for longer. It focuses on your assessed needs and goals.

What services are available under Support at Home?

ACH Group services are tailored to meet your unique goals and preferences, falling into three main categories – clinical supports, independence and everyday living

Your dedicated Care Partner at ACH Group will collaborate with you to choose the right mix of services from your approved list and adapt your support as your situation changes.

Clinical Supports

This includes professional health services such as nursing care, occupational therapy, and physiotherapy, as well as essential care management.

Independence

Get assistance with personal care like showering, dressing, taking medication, transport, or accessing short-term respite care to support you and your carers.

Everyday living

Receive help with domestic tasks such as cleaning, shopping, gardening, and preparing meals, making daily life easier and more enjoyable.

What contribution will I pay for Support at Home?

Under the Support at Home program, your contribution towards services is based on your individual income and assets assessment by Services Australia.

You only pay for contributions for the services you receive, rather than a set fee.

Contribution levels may vary depending on the type of service:
  • Clinical Support: No contribution.
  • Independence: Contributions typically range from 5% to 50% of the service cost.
  • Everyday Living: Contributions typically range from 17.5% to 80% of the service cost.

If you were a person receiving a Home Care Package on or before 12 September 2024, your contributions will not increase. You will continue to pay the same or less than under your previous package. This also applies to those who were on a Home Care Package before 1 July 2014 and have not changed providers.

Today ACH Group is at the Brighton Secondary School Thrive Expo, chatting with students about all the ways they can thrive in a career in aged care 💚

Students have been surprised to discover just how many career pathways exist in aged care, from care, nursing and allied health to hospitality, maintenance, business, and administration.

We’ve also been sharing how volunteering can be a great way to gain valuable experience and explore a field of interest, all while making a meaningful difference in someone’s day.

Thanks to everyone who stopped by for a chat, we loved hearing about your career aspirations and helping you discover the opportunities available at ACH Group!
