Support at Home with ACH Group

When choosing ACH Group as your Support at Home provider be assured you will be supported at each step. Our experienced team will work with you and those in your support network (if appropriate) to understand your needs and goals and design a care plan with you.

What to expect when onboarding

Throughout the onboarding process you will speak with our Welcome Team, followed by your dedicated Care Planner and Care Partner.

The Welcome Team

  • Understanding your assessed health care needs.
  • Understanding your Support at Home Agreement.
  • Understanding fees and charges.
  • Explaining your rights and responsibilities.
  • Ensuring initial services or supports are in place.
  • Advising next steps.

Your Care Planner

  • Co-designing a holistic package of care with supports and services to meet your individual health and care needs, within your funding.
  • Referral to allied health and nursing services for assessment, providing further recommendations for your care and services.
  • Providing detailed advice on the right Support at Home service mix for you.

Your Care Partner

  • Your single point of contact for any requests or issues.
  • Providing information, advice and referrals as needed.
  • Conducting an annual review to ensure your services and supports are meeting your needs and preferences.
  • Ongoing management of your package funds, and adjustment of services and supports if your needs change.

The culmination of the Joining Generations Intergenerational Program deserves a special moment of recognition - and our hearts are full! 💚

This celebration event brought together students, volunteers, families, and community partners to reflect on shared achievements and the incredible stories that highlight the power of connection across generations.

🙌 Finally, a very special acknowledgment to Brian Harris, whose vision started it all. A Charles Campbell College volunteer and passionate advocate against bullying and ageism, Brian brought everyone together and turned an idea into a truly meaningful community partnership - one that will continue to grow and expand across other schools and councils.
