If home is where your heart is, we can help you stay there.

ACH Group provides a comprehensive range of in-home services designed to help you maintain your independence and to stay in your current home.

With your care partner, a care plan is designed so you receive the appropriate level of support in a way that meets your current and future care needs, considering:

  • What you need to live well independently
  • Services to keep you safe at home
  • Ways to connect with your community
Whether you need help with your garden or a hand with your shopping, our services will help you stay in control of your own life.

How to get access home care services

The government provides two streams of funding to support you to stay living in your own home. These include the Commonwealth Home Support Program and Support at Home Program. If you are not eligible for government assistance, you can also choose to pay for services yourself (privately–funded). 

Support at Home Program

Support at Home is a new Australian Government initiative, designed to support older people live independently at home for longer through improved access to services, equipment and home modifications.

From 1 November 2025, the new Support at Home program will replace the Home Care Package (HCP) and Short-Term Restorative Care (STRC) programs.

Support at Home Program

From 1 November 2025, the Australian Government is introducing the new Support at Home program, a significant change designed to make accessing in-home aged care simpler, more flexible, and truly responsive to your unique needs. This program will replace the existing Home Care Package (HCP) Program and the Short-Term Restorative Care (STRC) Programme.

Commonwealth Home Support Program (CHSP)

Commonwealth Home Support Program (CHSP) is an ‘entry level’ program which gives you access to basic support services such as help around the home, assistance with shopping, respite, allied health, access to social experiences and more. ACH Group can help to access these government subsidised services, even lodging a referral for an assessment from My Aged Care on your behalf.

Privately fund help at home

Privately funded

You can access services immediately through private, self-funded services. There is no assessment by My Aged Care and you can access services when you want. This option is also suitable to those who want to access services while waiting for approval for government-subsidised services. There is no eligibility criteria and costs.

This weekend marks a major milestone for aged care in Australia, from 1 November, the new Aged Care Act and Support at Home Program come into effect, putting older Australians firmly at the centre of care.

At ACH Group, we welcome this important reform which strengthens protections, lifts expectations across the sector, and sets out clear rights for older people based on dignity, respect, and choice.

The introduction of the new Aged Care Act marks a significant shift for older Australians and the organisations that support them. For customers, the Act brings a stronger focus on clinical care, higher quality standards, and a new approach to financial contributions. ACH Group has been committed to guiding customers through these changes helping them understand the new requirements, supporting their planning, and ensuring continuity of care as the sector evolves.

Preparing for these changes has involved a significant transformation across our organisation, updating systems, enhancing governance, training our teams, and supporting our people to embed the new standards in their daily work.

We are proud of this collective effort across ACH Group and the broader aged care sector. Together, we are creating a safer, fairer, and more person-centred aged care system that upholds the rights, dignity, and choices of older Australians ensuring every individual receives care tailored to their needs under the protections and standards of the new Aged Care Act.
