Support with My Aged Care Assessment

Get the support that’s right for you

Getting the right level of support starts with talking about what matters to you and where you may need some extra help.

Before your My Aged Care assessment or reassessment, think about what has changed in your health or mobility, what you may be finding more difficult to do at home, and whether you are relying on family, friends or others for more help than you did before.

ACH Group can help you identify these changes and think about the support you may need so you feel prepared to discuss your needs with My Aged Care and get the right support to continue living well at home.

What is the My Aged Care assessment or reassessment?

Getting the right support starts with understanding what you need.

If things have changed, you may be finding some tasks harder, need more help at home or relying more on family and friends. A My Aged Care assessment or reassessment can help work out what support may be right for you.

ACH Group can help you prepare by talking through what has changed, what is important to you and where you may need some extra help.

What is a My Aged Care assessment or reassessment?

My Aged Care is the Australian Government’s aged care service. An assessment looks and your current needs and helps determine the government funded aged care services and support you may be eligible for.

If your needs or circumstances change, you may need a reassessment to make sure your support continues to meet your needs.

Your assessment or reassessment is a chance to talk about how you are managing at home and where you may need support.

What does an assessment or reassessment cover?

It may include:

  • Your health, mobility and overall wellbeing
  • Personal care and everyday activities
  • Household tasks and maintaining your home
  • The support you receive from family and friends
  • Social connection, transport and community participation
  • Your home environment and safety
  • Your goals and what you’d like to achieve

The aim is to build a complete picture of your circumstances, so the support available to you reflects your current needs.

Why is it important to prepare?

Many people gradually adapt to changes in their health, mobility or everyday activities without realising how much has changed. It’s natural to focus on what we can still do, rather than the extra effort, adjustments or support required to do it. For example, you might be taking longer to complete everyday tasks, relying more on family and friends, avoiding activities you once enjoyed, or doing things differently to stay independent.

When these changes aren’t discussed during an assessment or reassessment, the level of support recommended may not fully reflect your circumstances and needs.

Taking time to reflect on your day-to-day experience before an assessment can help you identify changes that may otherwise be overlooked and feel more confident discussing the support that could help you continue living well at home.

When should I consider an assessment or reassessment?

It might be time for an assessment or reassessment if:

  • Everyday tasks are becoming more difficult or taking longer
  • Your health or mobility has changed
  • You have experienced a fall, near miss or recent hospital stay
  • You need more help with personal care, meals or household tasks
  • Family members or friends are providing more support
  • You are getting out less or feeling less connected
  • Your home is becoming harder to move around safely
  • Your current services no longer meet your needs
  • Your goals or priorities have changed

Small changes can be easy to overlook. A simple question to ask yourself is:

What am I doing differently today compared with 12 months ago?

Think about what’s become harder, what you are doing differently and where you are relying on others for more help.
