How to access Home Care Services

You may be eligible for government subsidy through the Commonwealth Home Support Program (CHSP) or a Support at Home Program (SAH). You will require an assessment through My Aged Care (myagedcare.gov.au) to determine your eligibility and level of funding.

Commonwealth Home Support Program (CHSP)

The Commonwealth Home Support Program (CHSP) is an entry-level program designed to provide basic level care and support services. The Regional Assessment Service (RAS) will determine your eligibility and provide referral codes for your approved services. You are expected to co-contribute to the cost of your services.

Support at Home (SAH)

The new Support at Home Program (SAH) is designed for when you need more than entry level assistance, offering a coordinated approach to a greater range of services, aids, and equipment. Your eligibility and the level of support you receive will be determined by a comprehensive assessment, guiding the type and intensity of personalised care to help you stay at home for longer.

A century of courage, resilience and love! 💐🎉Happy belated 100th Birthday to the wonderful Margaret, who celebrated this incredible milestone with friends and staff at ViTA Residential Care Home last Tuesday! 🥳✨

The celebration was filled with joy - heartfelt wishes pouring in, a stunning bouquet of flowers, a sparkling birthday tiara, and a delicious cake to top it all off.

Linda Feldt, ACH Group CEO attended the celebration to personally congratulate Margaret and share in the heartfelt tributes from the ViTA community.

Margaret’s life story is nothing short of extraordinary. Her grandson John, who is also a Personal Trainer with ACH Group, shared:

“She was extremely brave moving to Australia as a young mother of five - leaving all their family behind with her partner to start a new life here for their children and future generations. Plus, in her younger years, Margaret served in the Women’s Land Army in England when she was just 16, working the jobs men had left behind when they were called to war - helping to keep food and ammunition production going.”

Margaret’s legacy shines brightly through the family she built with love - five children, 11 grandchildren, 17 great-grandchildren, and one great-great-grandchild. 💞
