ACH Group is excited to announce the winners for the third round of the 2025 ACH Group Way Awards!
The Good Lives Award recognises individuals or teams who bring our 4Rs (Reablement, Real Experiences, Right Relationships, and Roles) to life to support Good Lives for Older People.
🏆Congratulations to the Highercombe Team who were nominated for their exceptional commitment to supporting residents to live a good life through independence, connection, and holistic care.
In this particular instance, a resident arrived with complex care requirements, and needed full mobility support, extensive personal care assistance, and help to achieve a healthy weight. Through the dedication of Highercombe’s multidisciplinary team and the 4Rs framework, the resident regained mobility, restored his dignity, improved his health, and built meaningful social connections.
He ultimately returned home to live independently and continues to visit Highercombe, reflecting the genuine relationships formed. His journey showcases the team’s compassion and collaboration in delivering life-changing, person-centred care.
