1. Contact ACH Group on 1300 22 44 77
We will request that a representative from My Aged Care contacts you to start the process.

2. My Aged Care Assessment
My Aged Care will contact you to understand your needs and recommend services for you. This will include a phone screening and a face to face assessment, if required.

3. Ask for ACH Group
If you would like ACH Group to provide your service, simply tell your assessor and they will contact us to arrange. Alternatively, ask for a referral code and call us back on 1300 22 44 77.

4. Book Your Service
We will contact you to determine when you would like your service delivered.
